Thank you for choosing Dr. Alvarez as your healthcare provider. Our goal is to provide you with life-changing care every step of the way. To ensure we do that, please take a few minutes to familiarize yourself with our best practices.

To ensure we give all of our patients the highest standard of care, we will have to reschedule your appointment if we are unable to access your paperwork two weeks prior to your appointment.

To complete your new patient paperwork, please complete on ALL SIX of the forms below by clicking each link. In order to process your paperwork in a timely manner, please be sure to complete each section in its entirety. You will be able to submit each form individually.

We strongly encourage these forms to be filled out using a desktop or a laptop computer for ease of use, as the forms are not mobile and tablet friendly.
