We are honored that you continue to choose Dr. Alvarez as your healthcare provider. Our goal is to provide you with great care and service.

We value all of our patients. We want you to know what to expect from us at every step of your journey to a healthier, happier life. Please take a few minutes to familiarize yourself with the details below.  Don’t hesitate to get in touch if there’s anything you’re unsure of. You will be given a secure User ID and password that enables you to access your Portal. There you can view your personal and private documents, including lab and diagnostic test results, educational content, billing statements, and a whole host of other health information.

Items to Bring to Every Appointment:

  • Bring an updated medication list with you to all of your visits,

Make sure this includes the medication name, the dosage and the frequency that you take this medication. Please do include ALL medication and supplements that you are taking.

  • If you have your labs drawn from your primary care physician and/or another facility

Bring a copy of your labs with you to your visit. This way we can discuss any adjustments that need to be made on your behalf while you are here for your visit.

  • If you receive a new insurance card in the mail,

Bring it with you to the office. We are not aware of insurance changes and/or updates unless you tell us. It is your responsibility to let us
know so we may file claims to your insurance in a timely manner.

  • Medicare will be issuing new cards to all Medicare recipients,
This will replace your current claim number that you have on file with us. Per Medicare guidelines, we have until April of every year to make sure your Medicare identification numbers are updated in our system. When you receive your card, please make sure to bring it with you to our office so that we may scan it into our system.
  • Patients who are web-enabled, can chat with our office staff by messages, receive test results online, view upcoming appointment, etc. If you are not web-enabled and would like to be, please let one of our staff members know.

    **Please note, if you are web enabled, it is your responsibility to make sure that you check your patient portal for messages, this is how we will communicate to you*-

Please be aware that all copays and balances on your account will be due at the time of service, unless prior arrangement has been made with our office.

  • Schedule future appointments
  • Access your personal health record
  • View your current and past statements

By using the Patient Portal, you no longer have to call the office, leave a message, and wait for a response to get the results of your lab work; those results will be available to you through the Portal. • You can also send a message to the office through the Portal and expect a prompt reply.
