Welcome New Patients​

Thank you for choosing Dr. Alvarez as your healthcare provider. Our goal is to provide you with life-changing care every step of the way. To ensure we do that, please take a few minutes to familiarize yourself with the details below.

Your New Patient Packet: Please drop or mail your paperwork and records release back to our office at least 1 week prior to your appointment. We also politely request that you fill the paperwork out completely, including the Records Request, and submit the form to your previous physician. They can then fax your cardiac records to us so that we can get to know your specific needs. To ensure we give all of our patients the highest standard of care, we will have to reschedule your appointment if we are unable to access your paperwork 1 week prior to your appointment.

Items to Bring to Every Appointment:

  • Current insurance cards
  • Credit Card, Checkbook, or Cash for payments owed at the time of service
  • Current medication list: vitamins, supplements and any over-the-counter medication you’re currently taking

Arrival Time: Please make every effort to arrive 30 minutes prior to your appointment. This will give you ample time to complete the registration process and will allow you to update your demographic, insurance and health information if necessary.

Late Arrival Policy: If you arrive more than fifteen (15) minutes late, you will be asked to reschedule your appointment. If you are running late, please call and let us know as soon as possible.
